How to arrange pre-start health screening
After hiring a caregiver, you may choose to pay for any needed screening before her start date if she does not already have documentation.
Choose the screening you need

How new parents can coordinate the process
1. Confirm the screening scope
Agree with the caregiver whether she will complete a TB infection blood test (IGRA) only or add MMR and Varicella antibody testing before her start date.
2. Prepare the caregiver’s information together
Have the caregiver confirm and agree that the family will help. Gather her legal name, date of birth, ZIP Code, and an email address and phone number you both agree can receive notices. The patient must always be the caregiver, not the parent.
3. Have the family register, order, and pay
With the caregiver aware or present, a parent can operate the Ulta website. Choose only one listing above, select a quantity of 1, pick a collection site using her ZIP Code, and have the family cover the cost. Follow Ulta checkout for current pricing and payment options.
4. Print the paperwork and arrange the visit
After payment, download and print the PSC Requisition from My Orders, then follow the site’s appointment or walk-in instructions. The family can arrange transportation or accompany the caregiver. Bring the requisition and any requested photo identification.
5. Help download the report
After the result notice arrives, the family can continue helping the caregiver sign in and download the official PDF while keeping her informed about how it will be used. Check the items below and ask a healthcare professional about any Positive or Indeterminate result.
Need a complete employment physical? Ulta is not the right service
Ulta’s current workflow covers lab-test ordering, specimen collection, and results. It does not replace an in-person physical exam or provide vaccinations or a clinician’s signature on your health-clearance form. For those services, use an occupational health clinic that offers pre-employment physicals.

- 2Before booking, confirm that the clinic can examine a household caregiver, sign the form you bring, and complete any needed TB testing, titers, or vaccinations during the same visit.
- 3Have the caregiver bring photo ID, the blank form, available vaccination records, and prior TB results. Ask for the total self-pay price, then have the family cover the cost.
Call the clinic and ask
“I’m arranging a pre-employment physical for a household caregiver. Can your clinician perform the exam and sign the form I bring? Can any required TB test, titers, or vaccines be done at the same visit, and what is the self-pay price?”
Concentra is one nationwide example; services vary by center, so call first. The treating clinician decides whether a clearance can be signed and whether any follow-up testing is needed.
What the family should check after testing
If the family helped with registration and ordering, it can also help the caregiver sign in and download the official PDF when the result is ready. Check these three items:
- Name: matches the caregiver’s legal name on her identification
- Test: lists QuantiFERON®-TB Gold Plus or the panel you agreed upon
- Date and result: shows the specimen date and the laboratory-issued result
Do not decide fitness for work yourself if the result is Positive or Indeterminate; ask the caregiver to follow up with a clinician or TB clinic. A Negative TB result still does not replace a complete physical clearance.
